چکیده

From the Smith-Kettlewell Institute of Visual Sciences, Pacific Medical Center. This investigation was supported by Research Career Program Award No. K3 NB-14,094 from the National Institute of Neurological Diseases and Blindness; Public Health Service Program Project Research Grant No. NB-06038, and General Research Support Grant No. S SOI FR-0S566. bearing surface was fashioned from a 1-mm-wide steel rod. One end formed a slotted hoop which was placed around the optic nerve, in contact with the posterior pole of the globe. The rod was bent so that it passed under the muscle cone, around the globe, and ended in front of, but not in contact with, the cornea (fig. 1) . This arrangement exhibited a longitudinal stiffness of 500 g/mm. Freedom of movement was verified by manual pressure backward on the rod. A silk thread joined the rod to a cantilever spring connected to a Grass strain gauge displacement transducer. A suture was placed in the cornea at the anterior pole of the globe and connected to a second Grass transducer. Steel piano wire cantilever springs, 70 mm long and 0.3 mm wide were attached to the strain gauges to allow movement to be recorded with a spring constant of 10 mg per mm on an Offner chart recorder. In this way the total globe retraction produced by intravenous injection of succinylcholine could be

Histopathologic Changes of Extraocular Mus-cle Fibers after Injection of Botulinum Toxin

Background: Use of botulinum toxin is becoming a popular way in treatment of some forms of strabismus. To evaluate the morphological changes in the orbital surface layer muscles of the rabbit eye following a single injection of botulinum toxin. Methods: 10 white Dutch rabbits weighing 2–3 kg received either 10 units (group 1; n=5) or 20 units (group 2; n=5) of botulinum toxin into the superior r...

Incomitant strabismus: does extraocular muscle form denote function?

The paradigm that an "underacting" extraocular muscle is always atrophic or hypoplastic and that an overacting extraocular muscle should always be enlarged leads to inconsistencies with clinical observations. These include findings of "overacting" inferior oblique muscles, superior rectus muscle overaction or contracture syndrome, and normal extraocular muscle diameters in patients with apparen...

Histology and Physiology of the Extraocular Muscles

As described in Chapter 4, the extraocular muscles perform two functions: optostatic and optokinetic. The optostatic function requires that the muscles maintain a state of postural tonicity; the optokinetic function requires that quick, tetanic contractions be performed. These two contradictory functions are served by two different sets of muscles in the skeletal muscle system. Eye muscles, how...

The Suh Muscle Hook: A New Muscle Hook for Tight Extraocular Muscles during Strabismus Surgery

Purpose: To introduce an advanced muscle hook to decrease risks and create more feasible extraocular muscle surgeries for physicians. Case Report: Tight extraocular muscles during strabismus surgery could be extremely challenging even in experienced hands due to restricted globe rotation, limited view, and lack of muscle elasticity. Although a variety of surgical instruments have been introduce...
